LifeWorks Services uses multiple points of entry to successfully provide the resident with excellent quality care. ​At key continuums of care critical access points, LifeWorks Services will navigate the resident to be treated in place. The Advanced Practitioner model works to reduce readmissions by working with RN Navigators to follow you through the continuum of care coordination and case management​. LifeWorks Services provides concierge medicine to provide patients with personalized care and proactive wellness.
